With the coordinated care of your physician, we can provide you with the services of: Skilled Nursing -assess your care needs, dressing changes, management of infectious disease process, catheter care, tracheotomy care, veni-puncture, intravenous fluid/medication management, teaching to patient and family, referrals for home health aide services, referral and coordination with other community resources and care of the terminally ill. Home Health Aide -provide assistance with eating, bathing, dressing, personal hygiene and activities of daily living. Physical Therapist -assess and treat functional impairments that result from injury, stroke, surgery, fractures or other medical conditions. Medical Social Worker - assess emotional and social factors relating to the patient and family. Provides counseling and education, assistance in financial matters. Coordination of community resources.
