Care Management services: Homemaker services, respite care, legal aide, chore, residential assistance, minor home repair, medical trips, can assist patients to apply for SNAP, VICAP, and SMP. Home Delivered Meals are provided to elderly individuals who are unable to go to senior centers or other group meal sites. Individuals receive up to five meals per week. These meals may either be frozen, canned, or hot depending on dietary limitations and other considerations. For more information, call 276.783.8157.
How to get started:
Call to schedule an intake. Offers a variety of programs to services individuals 60 and over in the service area.
Serving:
At least age 60. Receive a qualifying score on a needs-based assessment. Be "essentially homebound." Care Management provides assessment of the seniors needs, works with individual and/or caregiver to identify and obtain services, equipment, and information through agency, community resources, or other sources that will allow the senior to remain independently in their home. Persons who receive care are monitored by home visits or telephone contact to determine if their needs are being met.
