How to get started:
Call MEOC and ask to speak with the intake worker for the program. Intake worker will reach out to patient and do a home visit assessment with the application.

Serving:
Can attend once as a guest, but must have assessment and be enrolled in the program if wanting a continuation of services. Must be age 60 or older.
